Please be informed of the following notifications to avoid PSC detention;
[Case 1]
○ Ship type : BULK CARRIER
○ Date : 2018. 4
○ Place : TARANTO, ITALY
○ Cause of Detention
- During the performance of fire drill only one firemen dressed appropriately to fight the fire
onboard. Missing simulation of shut-off of ventilation and cooling of bulkheads in fire area.
- During the abandon ship drill, crew member unable to lower lifeboat stb side and fail to bring
epirb and nautical chart from bridge. Furthermore, crew member failed to bring personal life
saving appliance during meeting at muster station on poop deck.
- Starboard side davit found seized and not able to lower at sea level lifeboat. Furthermore,
annual service result wrongly issued on June 09th 2018. Furthermore, rudder of Port side
lifeboat found seized and not operable. No documentary evidence found on board regarding
last renewal of steel wire falls of both lifeboats.
- N. 4 fire hoses in engine room found wasted and holed. Furthermore, n. 1 fire hose on poop
deck stb side found with leakages in way of connection coupling.
- N.16 metal empty drums found on main deck near cargo hold n.5. Furthermore, dismantled
exhausted parts of air condition system found at poop deck and garbage found collected on a
plastic container outside allowed garbage station.
- Annual survey has been carried out before the window of three months as per anniversary
date. In that case, the anniversary date shown on certificate shall be amended by
endorsement to a date which shall not be more than three months later than the date on
which the survey was completed.
- Engine room ventilation damper PT side found blocked. Furthermore, Galley Sup Vent and
steering room ventilation damper found blocked.
- Declaration of maritime labour compliance PART II, found not reviewed by competent
Authority or duty R.O.
- Following Emergency light found out of order: port side funnel, embarcation stb side, forward
liferaft.
- Aft Mast Head (lower) and stern (upper and lower) navigation lights found inoperative.
- Admiralty Nautical chart n. 1643 (Port of Taranto) and 187 for intended voyage not up to
date (intended voyage not already fixed). Furthermore, nautical charts n. 1439 and 176 for
the previous voyage (Misurata - Taranto) not up to date with NTM 289/17 - 985/17 - 446/17.
- Safety management audit by the Administration required before departure of the ship.
Deficiency(s) marked ISM is (are) objective evidence of a serious failure, or lack of
effectiveness, of implementation of the ISM Code.
[Case 2]
○ Ship type : REFRIGERATED CARGO SHIP
○ Date : 2018. 4
○ Place : LAS PALMAS, SPAIN
○ Cause of Detention
- Main deck access to E/R Door : Bent and doesn’t fit.
- Some air pipes closing device missing & holes closed with duct tape and painted.
- Beam joint to ceiling of E/R damaged. Some holes at structural parts on deck were found.
- Hole below shower room window (portside)
- Bow life raft lashed with cable and shackle.
- Officers toilet not flush water / urinal leaking / sink without pipe / sink not working.

○ Causes of Detention
- IMO number on E/R Bhd not marked properly in permanent way (partly).
- Bottom of escape trunk not partly insulated.
- ODME inspection, calibration not carried out by makers licensed company.
- Upper deck doors(P/S) not weather tight:
1) Door closing device broken
2) Not closed completely
- Fault alarm not indicated fire detecting system.

○ Causes of Detention
- Quick-closing valve on No.1 Generator fuel supply line found inoperative
- Manual valve found on the OWS sampling line
- NUC lamps position not confirmed with COLREG

- Access hatches to cargo holds not properly closed
- Closing device for access doors to bosun store malfunction
- Several fire dampers not operational
- Hath cover cleats with excessive clearance
- Fuel oil tanks sight glasses with rubber hoses temporally attached
- SMS implementation failed as evidenced by above deficiencies identified
[Case 11]

- Fire alarm panel out of order
- Hatch coaming securing cleats not working properly
- Fire damper of steering gear room not operational

○ Causes of Detention
- Suction pipe line for emergency fire pump in E/R has flange connection except for the flanged
connection to the sea inlet valve.
