o Causes of Detention
- Release gear lever for no.2 lifeboat reset incorrectly
- The sill of fire door for E/R emergency escape trunk seriously corroded and holed
- False records in oil record book

o Causes of Detention
- Lots of holes and fractures on hatch covers

o Causes of Detention
- Emergency generator unable to power emergency switchboard.
-> Load was not forwarded to ESBD during load test (sequence test) due to the trouble of
circuit breaker in ESBD.
[Case 4]
o Ship type : Oil Tanker
o Date : 2018. 4

o Causes of Detention
- Main form lines on deck leaked during test, and foam monitors leaked as well.
- Life boat (STBD) engine was not working. (RPM cannot be lowered after ‘full ahead’ has been
set.)

o Causes of Detention
- Both doors between wheel house and open deck are not secure against gas leaks.
- The emergency shutdown fuel oil v/v for No.1 G/E malfunctioned.
- Crew did not know the process for an emergency shut down of the machinery ventilator in
the emergency G/E room.

o Cause of Detention
- Emergency fire pump not working properly
[Case 8]
o Ship type : BULK CARRIER
o Date : 2018. 4
o Place : TARANTO, ITALY
o Cause of Detention
- During the fire drill only one firemen was dressed to fight a fire onboard. Failed simulation for
shutting off ventilation, and cooling of bulkheads in fire area.
- During abandon ship drill, crew members unable to lower lifeboat stb side and fail to bring
epirb and nautical chart from bridge. Furthermore, crew member failed to bring personal life
saving appliance to meeting at muster station on poop deck.
- Starboard side davit found to be seized and unable to lower lifeboat. Annual service result
wrongly issued on June 09th 2018. Rudder of Port side lifeboat found to be seized and
inoperable. No documentary evidence found on board regarding last renewal of steel wire falls
for both lifeboats.
- N. 4 fire hoses in engine room found to be wasted and holed. Furthermore, n. 1 fire hose on
poop deck stb side found to leak at connection coupling.
- N. 16 metal empty drums found on main deck near cargo hold n. 5. Furthermore, dismantled
exhausted parts of air condition system found at poop deck, garbage found collected on a
plastic container outside allowed garbage station.
- Annual survey has been carried out before the window of three months as per anniversary
date. In that case, the anniversary date shown on certificate should be amended by
endorsement to a date which should not be more than three months later than the date on
which the survey was completed.
- Engine room ventilation damper PT side found to be blocked. Furthermore, Galley Sup Vent
and steering room ventilation damper found to be blocked.
- Declaration of maritime labour compliance PART II, found not reviewed by competent
Authority or R.O.
- Emergency light found out of order: port side funnel, embarcation stb side, forward liferaft.
- Aft Mast Head (lower) and stern (upper and lower) navigation lights found to be inoperative.
- Admiralty Nautical chart (Port of Taranto) and 187 for intended voyage not up to date
(intended voyage not already fixed). Furthermore, nautical charts n. 1439 and 176 for the
previous voyage (Misurata - Taranto) not up to date with NTM 289/17 - 985/17 - 446/17.
- Safety management audit by the Administration is required before departure of the ship.
Deficiency(s) marked ISM is (are) objective evidence of a serious failure, or lack of
effectiveness, of implementation of the ISM Code.
[Case 9]
o Ship type : REFRIGERATED CARGO SHIP
o Date : 2018. 4
o Place : LAS PALMAS, SPAIN
o Cause of detention
- Main deck access to E/R Door : Bent and doesn’t fit.
- Some air pipes closing device missing and holes closed with duct tape and painted.
- Beam joint to ceiling of E/R damaged. Holes found in structural parts on deck.
- Hole below shower room window (portside)
- Bow of liferaft lashed with cable and shackle.
- Officers toilet not flushing water / urinal leaking / sink without pipe / sink not working.
[Case 10]
o Ship type : Oil/Chemical Tanker
o Date : 2018. 5
o Place : Gwangyang, Korea
o Causes of Detention
- IMO number on E/R Bhd not marked properly in a permanent way (partly).
- Bottom of escape trunk not partly insulated.
- ODME inspection, calibration not carried out by makers licensed company.
- Upper deck doors(P/S) not weather tight:
1) Door closing device broken
2) Not closed completely
- Fault alarm not indicated fire detecting system.

o Causes of detention
- Emergency generator not automatically connected to emergency switchboard.
