[Case 1]
o Ship type : Cargo Ship
o Date : April 2017
o Place : Himeji, Japan
o Causes of Detention
① The crew were unable to perform a satisfactory fire drill.
The crew failed two attempts to perform the fire drill, and were unfamiliar with procedures
to attack a fire, the donning of fire fighter clothes and operation of the breathing
apparatus.
② Rescue boat drill
The crew were unable to perform a satisfactory rescue boat drill.
Crew failed to perform the rescue boat drill and were unfamiliar with the procedures for
launching arrangements, the operation of the boat and the release/recovery of the hook.
③ Resources and personnel
Senior offices were not aware of their tasks, duties and responsibilities as demonstratede
by the inability to perform No.1 and & No.2 as listed above.
④ Rescue boat
Simultaneous release lever for hooks(fore and aft) were inoperable because the lever had
been cut off.

o Cause of Detention
- The sample sulphur content from the main engine was 0.65% (the maximum allowable
sulphur content: 0.10% within an ECA).
